چکیده

Background The mechanisms underlying hibernating myocardium are controversial. Whether de-oxygenation occurs at rest and/or stress in severely dysfunctional viable (i.e. hibernating) myocardium is currently unknown. By utlising the paramagnetic properties of deoxy-haemoglobin, blood oxygen level dependent magnetic resonance imaging (BOLD-MRI) can detect oxygenation and myocardial ischemia. We applied BOLD imaging to examine the pathophysiological relationship between coronary stenosis, regional wall motion abnormalities, ventricular scar and myocardial oxygenation in hibernating myocardium.

The value of myocardial perfusion imaging with Tc-99m MIBI for the prediction of perfusion improvement after percutaneous transluminal coronary angioplasty

Introduction: Percutaneous transluminal coronary angioplasty (PTCA) is an effective method for revascularizing of stenotic coronary vessels. Lack of response to this treatment, either in symptomatic or asymptomatic patients, is usually due to incomplete revascularization, restenosis, and/or irreversibility of myocardial perfusion. Introduction of a noninvasive metho...

The hibernating myocardium: current concepts, diagnostic dilemmas, and clinical challenges in the post-STICH era

A very large body of evidence—predominantly retrospective—suggests that revascularization is superior to optimal medical therapy in patients with a significant amount of ‘hibernating’ myocardium. Contemporary cardiological practice has embraced this standard of practice, as many centres worldwide place great emphasis upon the results of viability testing by non-invasive imaging techniques in de...

Myocardial oxygen tension during surgical revascularization. A clinical comparison between blood cardioplegia and crystalloid cardioplegia.

OBJECTIVE The aim of this study was to assess the effect of cardioplegic solutions on myocardial oxygenation during surgical revascularization. METHODS In 30 patients, randomized to receive crystalloid (CC) or blood (BC) cardioplegia, myocardial oxygen tension was measured continuously by polarography. RESULTS The two groups were comparable in terms of patients' age, sex, pre-operative ejec...

CARDIOVASCULAR MEDICINE Detection of scarred and viable myocardium using a new magnetic resonance imaging technique: blood oxygen level dependent (BOLD) MRI

Background: The identification of viable myocardium in patients with impaired left ventricular contraction secondary to coronary heart disease is important clinically as such myocardium is likely to benefit from revascularisation. Blood oxygen level dependent (BOLD) magnetic resonance imaging (MRI) relies on changes in deoxyhaemoglobin concentration under stress for signal generation and could ...
